In the screenshot on your original post, what is Facebook doing with that POST request to your game URL? And why is it trying to get a script from cdn.dsultra.com? It looks like some odd case of mistaken identity here.
I suggest that you work backwards from the app.facebook.com page to see what's wrong.

So my website provider check my website for viruses and injectors and etc and it came up clean.
Not sure where and why this "cdn.dsultra.com" script is loading when we attempt to test game on FaceBook?
I'll email the FaceBook people, maybe they have an answer...
